In most cases, a referral from your primary care physician is not required for an appointment with our specialists. However, the clinic requires specific information be gathered from previous physicians, surgical notes, ER notes, up-to-date imaging, and primary care records. Patients must sign a HIPAA release form to collect this information and maintain communication with all necessary primary care providers for the continuum of care.
